Payment
Payment is expected at the time of each appointment. Pay by cash, check, or online by Zelle or Chase Quick Pay mobile banking apps.
In-person or tele-medicine appointments
Both in-person and tele-medicine appointments via video conferencing are offered, depending upon patient's request.
Cancellation
Appointments cancelled with less than 24 hours notice will incur the full charge.
Insurance
I do not contract nor am I provider to any insurance companies. This allows me to focus on treating patients without the limitations mandated by insurance companies. It also enables me to spend my time with patients and not on the phone or completing forms online or at my desk.
